1.Print and complete the Prescription Form.*
* The prescription form is a PDF file, to download and print it you will need the Adobe Acrobat Plug-in.
2. Pack the Case
- Always pour alginate impressions in stone before shipping to the lab.
- Separate multiple impressions to prevent breakage.
- Secure bites, crowns or other small items in a small bag or container.
- Pack with foam and enclose the prescription.
- Make sure contents are secure and won't shift.
- Seal box securely with packing tape and attach air bill or mailing label.
3. Prepare the FedEx® Forms
- Sign the stamp.
- Attach the right portion of the stamp to your shipping box and keep the left portion as proof of shipment. Retain the left portion for your records or for tracking purposes. You may send as many cases per box as practical.
- Call 1-800-463-3339 and advise Customer Service that you have a pick-up. A representative will ask you for the time the package(s) will be ready and the account number to which you are billing. Your package is being billed to Oral Arts (the recipient).
- Call us at 1-800-354-2075 to arrange a pick-up.
When you need more stamps, call Oral Arts at 1-800-354-2075 or e-mail us. It is important that you order your stamps through our shipping department rather than calling FedEx directly. This will ensure that your pre-printed air bills are completed accurately so that your cases arrive as scheduled and all appropriate shipping expenses are billed back to Oral Arts.

Shipping Policy
Oral Arts pays the cost of shipping your cases both ways!
While many labs have instituted shipping charges at least one way, we believe that getting your cases to and from the laboratory still remains our responsibility. You have several convenient options when shipping your cases. Choose from our local courier service in areas where applicable, U.S. Mail or express service. We will provide you with complimentary air bills, packing materials and boxes. This policy does not apply to overnight deliveries or weekend pickups, where slight additional fees will be charged. Financing
Oral Arts offers you the convenience of a monthly open account on all of your casework. Invoices are sent with cases and statements are mailed on the last business day of the month. The statement balance is due and payable by the fifteenth of the following. A service charge of 1.5% per month (annual rate of 18%) will be applied to any unpaid balance. Any costs associated with collecting an unpaid balance, including legal fees, will be charged to the customer. Accounts with outstanding balances of over 45 days will be converted to a C.O.D. basis.
Warranty
Oral Arts Dental Laboratories guarantees that our work will be free of defects due to materials or workmanship. We also guarantee that the appliance will fit the provided model and be constructed to the design requested by the Rx. We will remake or repair any appliance at a reduced fee or at no charge depending on the circumstances of the case. This guarantee will be extended with proof-of-purchase, which includes an original invoice number/date, patient's name and the original model. All materials and workman-ship have a five-year guarantee. Oral Arts does not cover incidental or consequential damages, including but not limited to inconvenience, lost chairtime, costs incurred when removing or inserting the appliance, lost wages, pain and suffering or loss of profits. Oral Arts will only accept returns of implant parts on un-opened packages with the product in its original condition.
